Complex problems of total coxa arthroplasty and the possibility of physiotherapeutic intervention

This bachelor thesis is aimed at the complex total hip joint replacement problematics and its possibilities in physiotherapeutic positive influence. The theoretical part of this thesis describes the anatomy and kinesiology of the hip joint, implantation of the total hip joint replacement and subsequent rehabilitation. In the individual subchapters artificial joint classification, operating procedures, surgery cicatrix, patient education, contraindications, representative indications, functional malfunctions and its hip joint impact, preoperative and postoperative rehabilitation, physical therapy and rehabilitation from abroad perpective can be found. Quantitative research method was applied in the practical part. The representative sample compunded of 3 representatives. The most important criterion for them was to have had the total hip joint replacament done not longer than 3 days ago. The research included half-structured interview, input and output kinesilogical analysis and the collected data analysis. Each representative underwent 10 therapies, which were composed of my own excercise unit and physiotherapeutic procedures. The data analysis collected over those 10 therapies has shown that excercise unit of mine with the chosen physiotherapeutic procedures has improven those patients overall physical condition, has lower their lower limb swelling and its pain, has enhanced their joint scope, its active mobility and muscle strenght. This bachelor thesis might serve as either a useful resource of information regarding the total hip joint replacement problematics for professionals in this field or for patients awaiting or having underwet this surgergy. Last but not least current and future physiotherapist may also find this thesis useful in their work.
Physiotherapy in patients before intended hip joint replacement within the concept of prehabilitation

This bachelor thesis deals with the possibility of physiotherapy before the planned hip joint surgery within the concept of prehabilitation. In this thesis I deal with the possibility of preparing patients for this extensive operation. In the theoretical part, I process the knowledge of anatomy and kinesiology of the hip joint, then I outline the issue of the concept of prehabilitation, which is becoming an increasingly discussed topic abroad. In the next part, I provide information related to the total hip arthroplasty and complications during this procedure. Then the possibilities of preoperative physiotherapy are described in the thesis. The aim of this thesis was to record the preparation and describe the physiotherapeutic intervention in a group of patients before the planned hip replacement, for which I used qualitative research methods. The research unit consisted of one man and two women, therapy was usually performed 1-2 times a week for 60-75 minutes, for approximately two months. The data were collected by using an initial and final kinesiological analysis, an interview and an observation. According to that, a rehabilitation plan based on kinesiological and physiotherapeutic procedures and individually tailored to patients was developed. All probands received an exercise unit, which they practiced at home. All patients improved and two of the three patients underwent surgery without complications and the recovery progressed without problems. A third patient did not undergo surgery for epidemiological reasons. The work can inform the professional public, physiotherapists or students of physiotherapy about the possibilities of preoperative rehabilitation.

Results of the research realized through interviews show that women and men of different ages undergo joint puncture. Most often, patients come to the orthopedic outpatientś deparment with swelling, joint pain. Patients undergo joint puncture and corticoid appliacation intraarticularly.

The practical part presents a case study of a physiotherapy treatment after a total knee replacement. Method: This thesis consists of a theoretical and a practical part. The theoretical part describes the scientific findings on the subject of the knee joint from an anatomical and kinesiological perspective. Furthermore, this part deals with the gonarthrosis, the total knee replacement, and the physiotherapy treatment. The practical part shows the physiotherapy treatment of a female patient after a total knee replacement. The case study took place during an internship at the Military University Hospital (ÚVN). It contains a kinesiological analysis, a short - and a long-term physiotherapy schedule, a description of the therapy sessions as well as a final evaluation of the effects of the therapy. Title: A case study of the Physiotherapy treatment of gonarthrosis (arthrosis of the knee joint). Thesis aim: The following case study was conducted on a 70-year old woman in February 2012 with the diagnosis of gonarthrosis (arthrosis of the knee joint) at Revmatolicky Ustav. The aim of the thesis was to report the case study of gonarthrosis (arthrosis of the knee joint) and review knowledge about anatomy, neurology, kinesiology and pathology underlying the patients condition, and in addition an attempt to review the development of this diagnosis. The second part includes the patients anamnesis, kinesiological examinations and the therapy itself in details. Clinical findings: The patient recently was operated for the gonarthrotis with a total endoprosthesis, and pain, decreased range of motion and lack of muscle strength that usually follows after this was treated, and the treatment showed great progress in terms of development of the above mentioned. Title: The case study of the physiotherapeutic care of a patient with total endoprothesis of a knee joint Objectives: The purpose of this bachelor thesis is the processing of case reports about one patient during the bachelor's practice, which took place in the hospital Centrum léčby pohybového aparátu. This is a case study of the physiotherapeutic care of a patient with artrotic diagnosis with implantation of total endoprosthesis of the left knee joint. The patient was hospitalized on 9 January 2019 and the surgery took place on 10 January 2019. Methods: The thesis is divided into general and special part. The general part explains anatomy and kinesiology of the knee joint. Further, the thesis deals with the description of arthrotic disease, surgical treatment by application of endoprosthesis and physiotherapeutic care after surgery. The content of the special part is a case study of the patient. From physiotherapeutic methods, I used soft tissue techniques, Lewit's mobilization and postisometric relaxation, sensomotorcal stimulation by Janda, exercises according to muscle test and Kabat's method PNF. The differences in physiotherapy after implantation of total hip and knee joint replacement

This bachelor thesis deals with physiotherapy after the implantation of total hip and total knee replacement. The structure of the thesis consists of theoretical and practical part. In the theoretical part there is an anatomical build of the hip and knee joint, it also includes a description of movements that can be performed in the joints. Next subchapter is devoted to total hip and total knee replacement - types, indications and contraindications to their implantation or possible genesis of complications after these orthopedic operations. The specifics of rehabilitation of total joint replacement of the mentioned joints are sequentially described. The work also acquaints with various physiotherapeutic methods and concepts which are used in the postoperative phase of rehabilitation. The last section of the theoretical part contains a description of physical therapy options that can be used as a part of physiotherapy. The qualitative research method was used to process the practical part of the thesis. The research group consisted of two patients after total hip replacement and the same number of patients were observed after replacement of the knee joint in the posthospitalization phase of rehabilitation while they were staying at the Hamza Treatment Centre in Luže - Košumberk. Observations, semi-structured interview and kinesiological analyse (input and output) were used for obtaining the dates. Then the dates were analyzed and completed by a description of the course of the therapies throughout the patient´s staying in the rehabilitation facility. After that all the dates were processed in the form of case reports and evaluated. The bachelor thesis can serve as a comprehensive overview of common and different features in physiotherapy after the implantation of total hip and total knee replacement. It is intended for both physiotherapists and the general public.
Degenerative joint disease in Great Moravian population from the locality Mikulčice - Valy

This diploma thesis had two primary objectives. The first one was to examine if the incidence and development of primary degenerative joint disease (DJD) of the joints of the vertebral column and the limbs in an early medieval population from a cemetery by the sixth church in Mikulčice were affected mainly by the skeletal age of the deceased, or if other factor of physical loads was more significant. A set of 83 skeletal remains was evaluated primarily by Waldron's method (2009), but methods by Stloukal and Vyhnánek (1976); Buikstra and Ubelaker (1994); and Sager (1969) were also used. Schmorl nodes and eburnation were tracked too. Evaluated were vertebral joint (both intervertebral and apophyseal), jaw and limb joints. The results indicate that the incidence of degenerative joint disease in intervertebral joints, temporomandibular joint and appendicular joints is significantly affected by age, with the onset of degeneration being after the 40th year of skeletal age. No correlation of incidence of Schmorl nodes with age was present, and only a few cases of eburnace were noted. The relationship between the presence of DJD and the sex of the skeletons was not approved. I have compared these results with those from similar studies.
